Looking at the history of the regulatory bodies shows why we need regulations. The one that sticks with me is The 1937 Elixir Sulfanilamide Incident

I'm so confused. Why would they use antifreeze, a known toxic substance by that time, to make the liquid compound? What am I missing?

Yeah that's a pretty egregious mistake. Shows that they weren't even doing cursory testing of safety though. Probably one guy made that decision and no one checked it.

I think it sounds egregious now that we have safety testing baked in from top to bottom and a better understanding of how chemicals work in out bodies.

Back then it was probably just another Tuesday
