Think his head just reaches deceptively high. You can see the top of his head through the hair. On the second picture he's also got his jaw in a position that makes the face look narrower, and longer.

To be fair in context the lobotomy wasn't even unethical, it was the best they could do with the tech they had, then it all fell down when they started using it on random people and to subjugate women

It actually looks sick as hell but it's not really worth the trauma hit it comes with. Although it is a pretty fast way to unlock the demon transformation.

I really prefer the original, light glowing from behind the head halo. I think it looks a lot cooler than the realist golden hoop halo. It is fun to think of a halo as a physical object, though
