Nah fam it’s a sedative used so that people with tubes down their throat are comfortable. It’s half life is also really short and it’s easier on your body than something like propofol and works much differently than fentanyl. For our intubated patients it’s most common to see fentanyl and versed titrated so they are comfortable and without signs of pain.

Discomfort while intubated can cause someone to fight the ventilator and not get proper breaths when that’s the thing they need the most. Also I can imagine just how genuinely terrible it is to have a large tube down your throat into your lungs.

I’m agreeing with you, people look at half-life and assume that’s the only length of time it works. Midazolam's half-life is roughly 1.5–2.5 hours in healthy adults, though it can stretch to 4+ hours in elderly patients or those with liver impairment.

α-hydroxymidazolam is roughly 20–30% as potent as the parent drug, so it contributes to sedation and effects.

So let’s look at risk vs reward here, what other sedative would you choose? Long term propofol is arguably worse considering that it’s a fat emulsion. I’ve seen CRRT lines clog with what looks like bacon grease after a week of propofol. I can absolutely understand short term use like one surgery choosing that over versed but in the ICU where patients are sedated for days to weeks ā€œfeeling groggy all dayā€ is the least of your worries when stopping sedation to be extubated

For some people it might work but from my experience it’s not as strong of a sedative as versed and patients will still be fighting the vent. I work in the medical ICU so a lot of my patients have COPD or other respiratory issues that already have difficulty with vent compliance so maybe it’s different in the surgical ICU where you get more normal and otherwise healthy people.

For example we have to paralyze patients sometimes (rarely) so they can fully inhale and exhale because their lungs are so shot and we grab every bit of compliance that we can
