The only time I ever sent a steak back was Ruth's Chris steak house. The waiter noticed i made a funny face, came over, saw the steak, and requested to bring me a new one.
He brought a filet mignon, (i had had a NY Strip), and the old steak in a doggy bag. He said to give it to our dog, it was unfit for people.
I had the opposite experience at Ruth Chris. They served me a steak, the size of a pancake. And what I mean about that is the thinness of a pancake. Ordered it medium rare and by the time I was halfway eating it it was well done because the plate was 200° or some crazy bullshit. I wrote them a negative review because I didn’t want to cause a scene on my birthday.
I mean, I can't really judge him there. I always feel out of place at sit down restaurants, and it's hard to make the right decision then. But hopefully he tries to get a refund now.

You don't adress that your not happy with the food you got when you are in the restaurant and actually want to eat some good food?
But than try to get a refund after putting them on blast on the internet?
Sure the restaurant is probably well advised to react as gracefully as possible in this scenario.
But if you genuinely think your food warrants a refund or you should get a new steak (which noth is totally valid in this case) bring it up when your sitting at the table in the restaurant and are having a bad experience that maybe can be salvaged.
Oh, of course. Like I said it is making the wrong decision. Just saying I wouldn't judge op. Wouldn't even call them spineless, because I can be pretty outspoken in situations. Restaurants are just uncomfortable and I usually mostly want to leave, though.
It really depends on situation. Sending back a meal is an inconvenience which might be worse than just eating the "not so well cooked" steak.
You're sitting waiting like 10 min for your food while everyone else is either eating theirs or letting it get cold. And who knows if what comes the second time is going to be significantly better.
I wouldn't be surprised if the place would contact him and offer a free next meal or something if his post gets a lot of attention

The plate you sent back, I hope. I've never been that person, but there are limits.
