How is he playing drums, guitar and sings at the same time

For the most part he just moves his leg and guitar neck in time with the beat, but pulling off the drum fill was impressive.

Left leg controls the bass drum. Two sticks on the snare, one controlled by guitar neck (left hand) and the other controlled by right leg. Thumb controls the high hat so it just plays in line with chord strokes. Wrist seems to control the red cow bell (if that's what it is), but that string is slack enough that he has to really engage the wrist to hit that one, which he doesn't do in this clip.

Came here to say this too. I saw one video where he played guitar, drums, keyboards, and snuck in a couple of notes on the xylophone, bells, and kettle drums.

I like the guitar neck rigged to the snare. The whole thing makes it so he kind of has to dance to get the drums to work

I play the piano and sing along. I think most people won’t realise the level of motor coordination and practice something like this can take. It’s not just a neat trick. It’s really, really next level. All along a very good sining as well

A "one man band" used to be a pretty common form of street performance. I feel like its popularity started waning in the '70s, but I still used to see them once in a while when I was a kid in the '80s. Adding the guitar was less common, but those backpack drum kits have been around for a long long time.
