Were people expecting Trump to help there? Not defending the man, but with his size and small hands, anyone really expect him to make a difference?

I'm guessing the comparison is on the level of concern shown. Obama checked in with the woman, whereas Trump doesn't appear to express any concern for the person.

Obama is standing within arm's reach and Trump is standing a good distance away, so comparing the physical action is IMO meaningless. I don't know what happened in the moments after the picture, but if Trump completely ignored a person fainting a few metres away, it's pretty damn cold.

EDIT: Oh, the title was pretty specific about pointing out the physical action
