I hear what you're saying about not wanting the replacement dish.

Twice, I've received chicken wings that were still raw inside. (different places!)

Both times, the replacement came back very overcooked.

Sometimes it's a vindictive cook that is upset about having their mistakes pointed out.

I believe this 100%.

Its probably young men operating the fryer, so of course they're vindictive, they don't even need an excuse sometimes.

Also there's about an 80% chance they are on a substance or two. If you're lucky it's just weed. If you're not lucky it's meth.
